Note: you must have MS Excel installed on your computer to use this file.

It's Easy - Just Follow These Steps:

  1. Click the link above, and when prompted, "Save" the file. Do not try to open it directly from that link (you may not be able to save your order!). Depending on how your browser is set up, you may have to right click the link and select "save target as".
  2. Once downloaded to your computer, open the form in Excel - be sure to select "enable macros" when prompted.
  3. Fill out the form on the Door Order tab (see below) - make sure to fill out the contact information!
  4. Select the Fax Form tab (which is automatically filled in) and print (see below) .
  5. Fax it to us at 205-510-4233 or 800-277-0443 (you may also email the file to your Inside Sales Representative per their instruction).
  6. You should get a call from your Inside Sales Representative to confirm your order.
