Credit Forms
All transactions with HGH are on a cash basis (cash or major
credit card) unless otherwise arranged. You may download and
fill out the forms below to arrange for credit or check options.

and ask to speak to someone in the credit department.
Credit Application (PDF)
Download and fill out this form to apply for a line of credit
with HGH.
Note: if you submit the credit application,
you do not have to submit the Accept Check form or the Door
Order - No Deposit form below.
Credit Release Form (PDF)
Gives credit references your permission to respond to
our reference requests. This form is included if you download the Credit Application or the Accept Check Application.
Accept Check Application (PDF)
Download and fill out this form only to pay for orders or
invoices with personal or business check.
Door Order - No Deposit Form (PDF)
Download and fill out this form only to place door orders
without a deposit. (Door orders regularly require a deposit
of 50% for orders over $300, or 100% under $300.)
Delivery Without Signature Form (PDF)
Use this form if you would like your deliveries left at
the delivery location whether or not a person is available
to sign for receipt.
One-time EFT (Check by Fax/Phone) Authorization (PDF)
If you would like to have a payment on your account drafted
from your checking account, print this form and fill out
invoices/amounts, ATTACH A COMPLETED & SIGNED
CHECK, and fax back to one of the fax numbers
listed.
Sales Forms
Sherwood Custom Door Order Form (PDF)
Fax order form for custom cabinet doors - just print it,
fill it in, and fax it to us to order. (Electronic
form also available on the Sherwood
site.)
Homeowner/Retail Internet Instruction Sheet (PDF)
Instructions for how to use the homeowner/retail internet
account, with blanks for your retail account user ID and
password - this is for your customers. If you are uncertain
about this, check with your sales rep, or email sales@hghhardware.com.
